Federal Survey Cards will be sent home with students on September 7, 2011. Please complete the Federal Survey Card immediately and return it to the student's Advisory Teacher.

The survey is needed to identify federally-connected students and to secure other information required to qualify for impact aid funds under Public Law 103-382.

Please ensure that the survey card is signed and dated by the parent/guardian. If survey card is missing information, you will receive a call from your students Advisory Teacher.
